• Conduct comprehensive evaluations of members’ needs and eligibility using care management tools and data review.
• Identify high risk factors and service needs, recommending case resolution strategies and referrals as appropriate.
• Coordinate and implement care plan activities, monitoring progress and adjusting as necessary.
• Consult with case managers, medical directors, and health programs to overcome barriers and achieve optimal outcomes.
• Identify and escalate quality of care issues through established channels.
• Utilize negotiation and motivational interviewing to engage members and promote health changes.

Hours: Monday – Friday, 8am-5pm, no weekends, no holidays
Pay: $34.13 per hour

Hybrid Behavioral Case Manager Highlights

• Hybrid role with 50-75% travel to meet with members face-to-face in Illinois.
• Willingness and ability to travel extensively, with reliable transportation eligible for mileage reimbursement.
• Field-based position managing a full caseload of waiver members with quarterly in-person visits.
• Compliance with contractual requirements and regulatory guidelines.

Hybrid Behavioral Case Manager Responsibilities
