About the Role:

Life Consultants Inc. is seeking a highly motivated and compassionate Human Service Professional to join our team in Chesapeake, VA. As a Human Service Professional, you will assist in supporting individuals who are in transitional housing and/or in a crisis home who struggle with severe mental health diagnoses and who typically need support locating permanent housing.

Responsibilities

• Provide direct support and counseling to individuals facing various challenges
• Assist clients in setting and achieving personal goals
• Develop and implement individualized treatment plans
• Collaborate with interdisciplinary teams to ensure comprehensive care
• Advocate for clients' rights and access to resources
• Maintain accurate and up-to-date documentation of client progress
• Stay informed about current best practices and trends in human services
• Provide supportive counseling/crisis intervention services to individuals as needed
• Rotate between company facilities to include transitional housing and corporate office
• Transport client(s) to appointments and other required travel

Requirements

• Minimum of 2 years of experience in a similar role or degree in Human Services or related field
• Valid state licensure or certification as a Human Service Professional
• Strong interpersonal and communication skills
