Job summary

AMEDISYS HOME HEALTH is seeking a compassionate hospice aide to provide essential personal care and support to patients in their homes. You will assist with daily living activities, mobility, meals, and homemaking while following a nurse's plan of care.

The role emphasizes safety, dignity, and patient comfort. Requirements include a high school diploma, current CNA certification, CPR, and a valid driver’s license.

Qualifications

• High school diploma or equivalent.
• Current state CNA certification/registration.
• CNA experience within last two years.
• Current CPR certification.
• Valid driver's license and reliable transportation.

Responsibilities

• Provides personal care services to patients.
• Assists with ambulation, transfers, positioning, and daily living tasks.
• Plans and prepares nutritious meals; assists with feeding.
• Performs homemaking and environmental tasks as assigned.
• Observes and reports changes in patient status to nurse.
• Adheres to plan of care written by a registered nurse.
• Assists with prescribed exercises as directed by nurse or therapist.
• Performs simple urine tests and records results.
• Other duties as assigned.

Responsibilities

• Provides personal care services to patients.
• Applies safety principles and proper body mechanics to the performance of specific techniques of personal and supportive care such as ambulation of clients, transferring clients, assisting with the normal range of motion and positioning, and doing household chores.
• Plans and prepares nutritious meals, assists with feeding, as assigned.
• Performs homemaking and other environmental services as assigned.
• Observes, reports, and documents any changes in patient's status. If unable to perform a certain task, report to the primary nurse.
• Follow specific written instructions for personal care prepared by a registered nurse in accordance with the plan of care. Written instructions are to be received prior to the delivery of services.
• Assists patients with prescribed exercises as instructed by nurse or therapist.
• Performs and records simple urine tests for sugar, acetone, and albumin.
• Other duties as assigned.

Qualifications

• High school diploma or equivalent.
• Current state certification (or registration) as a nurse assistant as per state certification/registration requirements.
• CNA experience within the last two years.
• Current CPR certification.
• Valid driver's license, reliable transportation and liability insurance.
