This position is responsible for managing an assigned caseload of clients diagnosed with a mental health or substance use diagnosis and part of the Permanent Supportive Housing program. The incumbent assists individuals in accessing housing and related services, and strives to maximize client choice, self‑determination and decision‑making in planning and coordinating housing services. The role also develops, implements, and reviews service plans and collaborates with community resources to meet client needs.
