Job summary

Coos Health & Wellness is seeking a dedicated Mental Health Associate to join the Adult Team as a supportive housing case manager. You will provide community-based services to help individuals achieve recovery and housing goals, in partnership with the North Bend Housing Project.

Responsibilities include case management, housing stabilization, skills training, and community integration to promote stability.

Qualifications

• Must be a Qualified Mental Health Associate or eligible.
• Bachelor's degree in health/behavioral health or 3 years of mental health experience/education.
• Valid driver’s license required.

Responsibilities

• Provide case management and recovery-oriented services.
• Deliver housing stabilization and skills training.
• Coordinate with families, medical and behavioral health providers.
• Apply Housing First principles to promote stability.
• Use motivational interviewing and stage-appropriate interventions.
• Complete clinical documentation meeting standards.
• Collaborate within a multidisciplinary team.
• Assist clients in accessing benefits and community resources.
