Job summary

Eliot Community Human Services, Inc. is seeking a part-time Housing First Coordinator for its Low Threshold Housing Team.

The role provides onsite case management, recovery coaching, and wrap-around support including MAT/psychiatry, medical consultation, and trauma-focused treatment modalities. Successful candidates will have a strong commitment to recovery, housing first philosophy, and knowledge of area resources.

Qualifications

• Bachelor's degree in human services or related field is required.
• Candidates with lived experience with substance use disorder and 2 years of sustained sobriety are encouraged.
• High school diploma/GED and at least one year of paid or volunteer experience with adults with mental illness and/or substance abuse.
• Must commit to substance use recovery and housing first philosophy.

Responsibilities

• Provide practical help, advocacy, and direct assistance to help clients gain daily living skills.
• Promote health and wellness by helping clients access medical care, HIV resources, nutrition, and exercise.
• Work with the multidisciplinary team to guide residents through legal concerns.
• Intervene in potential crises using trauma-informed de-escalation and escalate to leadership when needed.
• Educate clients on harm reduction principles and access to safe injection supplies.
• Engage in client-centered harm-reduction and Housing First framework.
• Develop and implement safety response plans as needed.

Eliot's Homeless Services Division is seeking a dedicated part-time Housing First Coordinator to join our Low Threshold Housing Team which provides an array of onsite supportive services, including recovery coaching, MAT/Psychiatry, wrap-around case management & clinical coordination, medical consultation and direct interventions, and access to trauma-focused treatment modalities.

In this role you will provide onsite case management & support in a highly collaborative fashion with residents. Successful candidates will have unwavering compassion, as well as an open mind in terms of pathways to recovery/harm reduction with respect to substance use disorders. Working knowledge of area resources/providers, housing navigation, and tenancy preservation are a plus. Candidates with lived experience are strongly encouraged to apply.

Responsibilities:

• Provide practical help and mentoring/modeling, advocacy, coordination, side-by-side individualized support, problem solving and direct assistance to help clients obtain skills needed for daily independent living.
• Promote concepts of health and wellness by assisting clients with accessing information and services related to medical care including HIV care resources, nutrition, and exercise as appropriate.
• Work within the multidisciplinary team to guide residents through legal concerns.
• Intervene in potential crisis situations, utilizing trauma informed, verbal de-escalation techniques - elevate concerns to leadership/on call personnel in real time as applicable.
• Educate and support clients around harm reduction principles and utilization, including access to safe injection supply kits when requested.
• Engage in client-centered/harm-reduction/Housing First framework.
• Develop and implement safety response plan as needed

Qualifications:

Schedule:Sunday 7am-3pm, Monday 5pm-9pm, Thursday 5pm-9pm, Friday 3pm-9pm and Saturday 3pm-9pm (28 hours)

Compensation:

• BA $33,600.00
• MA $35,000.00
