AT HOME CARE PHYSICIANS PLLC in Warrenville, IL is seeking a compassionate House Call Physician to deliver high-quality medical care to patients in their homes. The ideal candidate will have an MD or DO degree, an active Illinois medical license, and strong clinical judgment, prioritizing patient relationships and quality care.

Qualifications

• Reliable transportation and valid driver's license.

Responsibilities

• Conduct comprehensive in-home medical evaluations and treatment.

• Maintain accurate clinical documentation and compliance.
• Communicate treatment plans to patients and caregivers.

Responsibilities:

• Conduct comprehensive in-home medical evaluations, diagnoses, and treatment for a diverse patient population
• Develop and manage individualized care plans tailored to each patient's needs and medical history
• Coordinate with specialists, home health agencies, and care teams to ensure continuity of care
• Perform medication management, chronic disease management, and preventive care during home visits
• Maintain accurate and timely clinical documentation in compliance with regulatory standards
• Communicate effectively with patients, families, and caregivers regarding treatment plans and health goals
• Respond to urgent patient needs and provide appropriate follow-up care

Requirements:

• MD or DO degree from an accredited medical school
• Active and unrestricted Illinois medical license
• Board certified or board eligible in Internal Medicine, Family Medicine, or Geriatrics
• Valid DEA license and ability to prescribe medications
• Experience in primary care, geriatrics, or home-based medicine preferred
• Strong clinical judgment with the ability to work independently in a field-based setting
• Excellent communication, empathy, and patient rapport-building skills
• Reliable transportation and valid driver's license required
