Rochester Regional Health in New York’s St. Lawrence Region is seeking a Hospitalist Advanced Practice Provider (NP/PA) to deliver high-quality inpatient care. You will perform admissions, rounds, and discharge planning in a collaborative team setting with physicians and multidisciplinary staff.

The role emphasizes autonomy with support from a large system, with opportunities for professional growth and a focus on patient-centered care in Canton-Potsdam and Massena Hospitals.

Formación

• Graduate of NP/PA program
• Current NYS licensure or eligibility
• National certification as NP/PA
• Hospital medicine, internal medicine, critical care, emergency medicine, or inpatient experience preferred

Responsabilidades

• Perform admissions, patient evaluations, daily rounding, and discharge planning
• Manage acute and chronic medical conditions in collaboration with physicians
• Coordinate care with specialists, nursing, case management, rehabilitation and ancillary staff
• Participate in quality improvement and patient safety initiatives
• Develop strong relationships with patients and families ensuring continuity of care during hospitalization

Qualifications

• Graduate of an accredited Nurse Practitioner or Physician Assistant program
• Current New York State licensure or eligibility for licensure
• National certification as an NP or PA
• Hospital medicine, internal medicine, critical care, emergency medicine, or inpatient experience preferred
• Strong clinical assessment, communication, and decision-making skills
• Ability to work independently overnight while collaborating with physicians and multidisciplinary teams

In addition to the base pay range, certain provider positions may be eligible for variable compensation in accordance with health system pay practices.
