Job summary

Concord Hospital Health System is seeking an Advanced Practitioner to join the Hospitalist Program. The role provides inpatient acute care, including initial encounters, daily rounds, consultations, and patient education under physician supervision.

Required qualifications include NH licensure, a DEA license, ACLS, and board certification (NCCPA/ANCC/ANP). Prior provider experience and the ability to work in a collaborative team are essential.

Qualifications

• Completion of an Accredited Physician Assistant Program or Advanced Practice Nursing Program.
• Current license to practice in New Hampshire
• Current DEA License
• Board certification by NCCPA, ANCC, or AANP
• Current ACLS certification
• Experience as a provider is required

Responsibilities

• Obtains patient health history, performs physical examination, develops a differential diagnosis.
• Orders, interprets, and follows up on diagnostic tests.
• Diagnoses acute and chronic medical problems within the scope of practice.
• Prescribes and manages medications within practice standards.
• Provides patient education related to preventive care.
• Documents in the medical record according to department standards.
• Communicates effectively with the multidisciplinary care team.
• Performs medication reconciliation on admission and discharge to ensure safety.
