MultiCare Good Samaritan Hospital in Puyallup, WA, is seeking an entry-level Hospital Social Worker Trainee for a full-time day-shift role. The position focuses on cross continuum care coordination, psychosocial assessments, and interventions to improve patient outcomes.

You will engage with patients, families, and care teams, coordinating transitions of care and discharge planning. Required is a Master’s degree in Social Work with healthcare practicum.

Qualifications

• Licensed in Washington as LSWAIC or pursuing LICSW within four years.
• Hospital or equivalent experience preferred.

Responsibilities

• Perform cross continuum care activities across clinics, provider practices, urgent care, and other sites as appropriate.
• Provide customer service to the patient, family, physician/APP, and care team members through discharge milestones and progression to next level of care.
• Model and orchestrate consistent, professional communications and maintain proactive communication with colleagues and staff.
• Delegate responsibilities appropriately but remain accountable for outcomes.
• Ensure that the patient and family experiences medically appropriate transitions of care.
