Job summary

Care New England Health System seeks a Clinical Social Worker to provide psychosocial assessments, crisis intervention, counseling, and care coordination for patients and families within the hospital setting.

You will collaborate with an interdisciplinary team, participate in rounds, and help develop discharge plans while maintaining HIPAA-compliant documentation and advocating for patient needs.

Qualifications

• MSW is required.
• RI LCSW or LMHC licensure required; LICSW preferred.
• Experience in healthcare or hospital setting preferred; knowledge of NASW code of ethics.

Responsibilities

• Provide direct clinical social work services including psychosocial assessment, crisis intervention and referrals.
• Collaborate with interdisciplinary team; participate in rounds and contribute to patient care decisions.
• Maintain thorough documentation in patient records; complete required reporting.
