Job summary

Parkland Health is seeking a Social Worker (LMSW) to perform psychosocial screenings and assessments, develop and implement care plans, and coordinate resources to support patients and families. The role requires a Master’s degree in Social Work and Texas licensure, with CPR and SAMA certifications as specified for relevant cost centers.

The candidate will work with a multidisciplinary team to ensure timely and appropriate post-discharge arrangements.

Qualifications

• Master's degree in social work.
• Must be licensed by the Texas State Board of Social Work.
• Heartsaver CPR certification within 60 days for selected cost centers and SAMA certification prior to first clinical shift.

Responsibilities

• Performs psychosocial screenings and assessments to identify patient needs and plan care.
• Counsels patients and families on emotional, social, and financial aspects of illness.
• Participates in discharge planning and post-hospital placement planning.
• Communicates with the interdisciplinary team about discharge plans and resources.
• Documents findings and ensures compliance with policies and procedures.
