Job summary

SSM Health DePaul Hospital – St. Louis is seeking a Social Worker to provide social work assistance to patients and families in relation to social determinants of health.

This regular, full-time role is in the Social Services department with a weekend-option schedule that runs Friday through Monday. The position collaborates with a multidisciplinary team, coordinates community resources, and supports treatment planning.

Qualifications

• Master's degree in Social Work required.
• No experience required.

Responsibilities

• Interviews patients, assesses priorities and documents case activity.
• May consult with multi-disciplinary team to support patient’s treatment.
• Assists patients/families with obtaining community assistance by referral to proper resources.
• Develops and maintains relationships with community health, welfare, and social agencies to assist patient needs.
• Active participant with team regarding opportunities for improvement in standard work.
• Works in a constant state of alertness and safe manner.
• Performs other duties as assigned.
