Northwell Health seeks a social worker to deliver direct services to patients and families across care settings. You will use psychosocial assessments, provide counseling, crisis intervention, and assist with discharge planning to maximize patient functioning.

The role includes assigning and supervising social work students, collaborating with an interdisciplinary team, and ensuring thorough documentation in line with hospital policies.

Qualifications

• Master of Social Work degree required.
• Current NY MSW or Clinical Social Worker license required, plus certifications as needed.
• 1–3 years of relevant experience required.

Responsabilités

• Provide direct social work services to patients and families using varied modalities.
• Identify patients in need of assessment; advocate to improve access to care.
• Share assessment findings with interdisciplinary teams and contribute to care planning.
• Provide psychoeducation, counseling, and crisis intervention.
• Develop and implement treatment and discharge plans aligned with site policies.
• Document care in accordance with site policies; participate in committees as needed.
• Supervise/coordinate social work students; complete evaluations as required.
• Consult with program management on training and placement progress.
