Job summary

Holy Family Medical Center in Des Plaines, IL, is seeking a Case Manager for the Case Management department. This full-time daytime role (8am-4:30pm) supports patients and families through emotional care and coordinated discharge planning.

You will conduct psychosocial assessments, work within a multidisciplinary team, and document client records throughout hospitalization or treatment. Prior inpatient/outpatient behavioral health experience is preferred.

Qualifications

• Bachelor’s degree in Social Work, Marriage and Family Therapy or related field.
• Current registration or intern status with Board, as appropriate to education and licensing requirements.
• Behavioral Violence Prevention within 3 months of hire and maintained annually.
• Minimum 1 year inpatient or outpatient behavioral health experience preferred.

Responsibilities

• Provide emotional support, crisis intervention and counseling.
• Information and referral; referrals for financial assistance and supportive services.
• Complete psychosocial assessments and assist in Treatment Plans and discharge planning.
• Maintain clinical records for each client during length of stay.

Education

Bachelor’s degree in Social Work or related

BCLS (AHA) certification

Masters in Psychology or Social Work or LPC

Licensed Professional Counselor or related
