Job summary

TEEMA Solutions Group is seeking a pharmacy professional to support medication reconciliation and pharmacy operations during the patient hospital stay and at discharge.

Responsibilities include reconciling home medications with inpatient orders, assisting with high-risk patient discharge reconciliation, identifying discrepancies for providers, collecting and analyzing reconciliation data, and supporting pharmacy staff under pharmacy leadership in a large acute care setting.

Qualifications

• IV experience preferred.
• Epic experience required.
• 3+ years hospital pharmacy experience preferred.

Responsibilities

• Reconcile home medications with inpatient medication orders.
• Assist with medication reconciliation for high-risk patients at discharge.
• Identify and communicate medication discrepancies to providers.
• Collect and analyze medication reconciliation data.
• Support pharmacy staff under the direction of pharmacy leadership.
