Job summary

Endeavor Health in Elmhurst is seeking a Pharmacy Technician Med Reconciliation to support the patient admission process. You will collect Home Medication History, interview patients, and coordinate with pharmacies to document accurate information.

This role operates mainly in the Emergency Department under the supervision of a licensed pharmacist. The position is full-time with rotating evening shifts (11am–9:30pm or 12pm–10:30pm) and 40 hours per week, offering premium pay and a pathway for

Qualifications

High school diploma or GED required.
Current Illinois Pharmacy Technician license required.
Must obtain CPhT within 90 days of hire or have PTCB/ExCPT certification pending.

Responsibilities

Collect Home Medication History from patients admitted to the hospital.
Interview patients, contact pharmacies, and document information accurately.
Work primarily in the Emergency Department under supervision of a registered pharmacist.
