Qualifications

Job description

We are looking for an experienced Pharmacist to oversee Pharmacy operations at Bristol Health, ensuring accurate dispensing, patient‑focused pharmaceutical care, and compliance with regulatory requirements.

Essential Job Functions and Responsibilities

• Oversee all aspects of Pharmacy operations, ensuring accurate dispensing and improve processes to increase efficiency and reduce expenses.
• Ensure all medications are accurately prepared, labeled and checked before leaving the pharmacy.
• Interpret physician orders, accurately enter and validate orders; validate/enter all STAT orders within 15 minutes and all non‑urgent orders within 1 hour.
• Participate in the Pharmacy Improvement Process.
• Contribute to Departmental cost‑saving initiatives; submit ideas to the Pharmacy Manager.
• Ensure all chemotherapy preparations are checked by one Pharmacist and two other Pharmacy personnel.
• Resolve Pyxis issues.
• Supervise Technicians, signing all checked work and providing corrective action as needed.
• Maintain security and perpetual inventory of controlled substances.
• Ensure all IV admixtures are prepared using aseptic technique.
• Under guidance of the Pharmacy Manager, oversee training of new staff and students.
• Monitor inventory and order medications as needed to meet patient needs.
• Participate in the control, use and monitoring of investigational drugs.

Pharmaceutical Care Responsibilities

• Evaluate the patient’s drug therapy, clarifying dosage, drug interactions, allergies, contraindications and therapeutic duplication with the prescriber.
• Monitor and complete adverse drug reaction reports.
• Investigate and complete medication occurrence reports.
• Evaluate patient care and response to pharmacotherapy, including renal dosing, IV to PO conversion, aminoglycoside and vancomycin dosing, enoxaparin and epoetin‑alpha conversion, pediatric weight‑based dosing, antibiotic dosing and selection; document interventions.
• Monitor laboratory values and ensure TPN appropriateness; verify that LIP is credentialed.
• Evaluate chemotherapy regimes, verifying regimens/dosing against diagnosis.
• Actively participate in at least one committee.
• Discourage use of patient‑owned medications and non‑formulary medications; complete a non‑formulary form for each non‑formulary medication.
• Monitor warfarin therapy on each assigned patient and provide education when required.
• Provide evidence‑based drug information and teaching to patients and customers.

General Responsibilities

• Function as a preceptor to new employees and students.
• Complete all mandatory educational assignments on time.
• Attend staff meetings while on duty; sign off on minutes if absent.
• Read email and communication log daily; initial communication log.
• Conduct a self‑assessment, develop professional practice goals and demonstrate progress to completion of those goals.
• Voluntarily participate in the education of professional staff, including case presentations as appropriate.
• Function as liaison to assigned patient‑care location, interacting with managers and staff to promote customer satisfaction.
• Maintain less than six unexcused absences as per Hospital policy.
• Limit personal cell and internet use.
• Arrive to work on time and not depart early without approval.
• Practice Pharmacy in accordance with State and Federal statutes/laws.
• Perform all job functions assigned by the Pharmacy Manager or Clinical Coordinator.

Educational / Minimum Requirements

• B.S. degree in Pharmacy; Pharm.D. preferred.
• Previous hospital pharmacy experience preferred.
• ASHP‑accredited residency and/or Board Certification highly desirable.

State/Federal Mandated Licensure or Certification Requirements

• Current State of Connecticut license as a Registered Pharmacist in good standing.

Bristol Hospital Mandated Educational Requirements

• General orientation at time of hire.
• HealthStream and Communication with Deaf and Hearing Impaired training.
• Any additional educational requirements as determined by Pharmacy, Education or Human Resources.

Other Requirements

• Computer skills related to pharmacy operations.

Physical Requirements

• On feet for extended periods.
• Periodic stooping, bending and walking.
