Qualifications

• Assesses all physician orders for accuracy, allergies and interactions.
• Counsels patients on medication use on discharge or at need.
• Collaborates with nursing and physicians to optimize therapy.

Responsibilities

• Review antibiotic therapy for appropriateness and cost-effective alternatives.
• Direct technicians in IV admixture, ICU medications, TPN and chemotherapy.
• Attend codes to provide pharmaceutical expertise in emergent situations.
• Ensure proper storage/handling and monthly inspections of medications.
• Provide drug information to patients, nurses and physicians.
• Reorder stock when low and perform narcotic inventory per protocol.
