Requirements include a PharmD from an accredited college, eligibility for licensure in the applying state, and about 2 years of hospital experience.

Qualifications

• Pharmacy degree from an accredited College of Pharmacy.
• Prefer 2 years experience in a hospital setting.
• Pharmacist licensure in state applying or eligibility for licensure via examination or reciprocity.

Responsibilities

• Coordinate daily provision of high quality pharmaceutical care to hospital patients in compliance with federal and state laws, accreditation, and professional standards.
• Involve with multiple drug distribution functions including order entry, prospective review, and IV admixture preparation.
• Engage with physicians, nurses, and healthcare staff to clarify orders and document intervention outcomes.
