Job summary

Salem Health is seeking a pharmacist to provide expert medication management for hospitalized and ambulatory patients, supervising staff and collaborating with clinical teams to optimize therapy.

The role includes supervising pharmacy staff, delivering educational programs, and participating in quality initiatives. The position requires 24-hour float coverage across four rotating 10-hour shifts to support patient care and team needs.

Qualifications

• Doctor of Pharmacy degree or a Bachelor of Science in Pharmacy.
• Oregon pharmacist licensure required.
• BLS certification obtained within two months of hire.
• ACLS certification obtained within two months of hire.

Responsibilities

• Evaluate patient medical records to ensure safe and effective medication therapy.
• Verify provider medication orders for accuracy and appropriateness.
• Monitor and manage ongoing medication therapy with physicians for optimal results.
• Provide medication information and pharmacokinetic dosing guidance to clinical staff.
• Supervise and direct pharmacy staff, including technicians and students.
• Serve as liaison to nursing units and departments; manage inventory and education.
• Participate in quality management initiatives, rounds, and hospital committees.
• Maintain ongoing professional development and competence.
