Qualifications

• Active Occupational Therapist license in the state of practice.

• Develops and implements individualized treatment plans for patients.
• Evaluates patients using data from records, interviews, and observations.
• Provides direct occupational therapy services across age groups.
• Documents evaluation results and progress notes per department standards.
• Communicates with patients, families, physicians and team members about status and needs.
