Job summary

Wellstar Health System in Georgia is seeking a Hospital Pharmacist for an Evening shift. The role blends patient-centered pharmaceutical care with traditional medications management, including antibiotic dosing and warfarin management, while ensuring compliance with Georgia pharmacy law.

You will educate nurses and physicians, review regimens, prepare IV admixtures, and participate in rounds to optimize therapy and safe discharge counseling.

Qualifications

• BS or Pharm.D. degree from an accredited school of pharmacy is required.
• PhD? actually PharmD preferred and PGY1 residency preferred.
• Licensed pharmacist with Georgia registration; BCPS preferred upon hire.

Responsibilities

• Evaluating and monitoring drug regimens for appropriateness.
• Interpreting physician orders and screening them for interactions.
• Providing patient and nursing education when requested.
• Reviewing medication profiles to maintain accuracy and optimize therapy.
• Able to perform all duties of a Pharmaceutical Care Pharmacist General Pharmacy Practice.
• Preparing IV admixtures, compounding, and repackaging a wide variety of pharmaceuticals.
• Entering and verifying orders into the pharmacy computer system, and proper dispensing of medications.
• Able to follow patients for antibiotic and anticoagulant dosing.
• Participates in daily rounds and collaborates with healthcare providers on patient education.
• Assists with medication reconciliation and discharge counseling.

Education

B.S. from an accredited School of Pharmacy.

Pharm.D. from an accredited college of pharmacy is Preferred

PGY1 Pharmacy Residency is Preferred
