Job Description:
Essential Duties & Responsibilities including but not limited to: ·
  • The Hospital Based Admissions Nurse determines appropriateness of General Inpatient (GIP) Level of care, coordinates and conducts preadmission activities · Increases accessibility to hospice care, to manage the hospice admission process via effective communication · Describes and explains hospice services to a potential patient/family and elicits responses to questions regarding patient/family attitude toward hospice care, physical and psycho/social/emotional status, in order to put the patient and family at ease about choosing hospice and fully inform them about the program · Gathers pertinent information necessary for admission · Speaks to patient/family about insurance coverage and financial obligations, using knowledge of hospice fees, cost and reimbursement sources · Promote hospice services, to ensure appropriate GIP referrals to hospice program to discharge planners, physicians, other health care providers and referral sources · Manage care of patients, coordinate all services and facilitate communication between all providers. · Assess patient and coordinate cares with hospital care team including MD, Case Manager, RN and hospice team. · Provides expert symptom management and recommendations in coordination with the Hospice Medical Director · Maintain productivity expectations. · Provide documentation of services utilizing problem-oriented record. · Plan for continuity of care with nursing team members, patient, family, and other disciplines. · Use opportunities for teaching nursing care, comfort measures and health practices to the patient, families and hospital staff. · Responsible for daily nursing visits and assessment of ongoing eligibility of patients admitted to GIP level of care. Coordinates with the hospice IDT if the patient becomes no longer eligible and an alternative plan becomes necessary. · Communicates and coordinates with other health and social agency personnel to assist in planning, implementing, and evaluation of services to the patient and family. Patient Care Assumes responsibility to coordinate patient care for assigned patients including but not limited to: · Completion of Hospice Outcomes and Patient Evaluation (HOPE) time points. Accepts accountability for patient/ family satisfaction scores and quality of care outcome measure results. · Completes and documents an initial assessment of patient and family, as assigned, to determine GIP eligibility and hospice care needs. Provides a complete physical assessment and history of current and previous illness(es). Develops a patient specific plan of care, which establishes goals based on nursing diagnosis and incorporates therapeutic, preventive, and palliative nursing actions, with emphasis on pain and symptom management. Includes the patient, family, and hospital care providers in the planning process. · Initiates the plan of care, as assigned, and makes necessary revisions as patient status and needs change. Recommends medications and treatments as prescribed by the physician. · Counsels the patient, family and hospital care providers in meeting patient needs related to physical, emotional, and spiritual needs
Minimum Qualifications:

Education: · Registered Nurse from a Graduate of National League for Nursing accredited school of nursing. · BSN strongly preferred.

Licensure, Certification
Registration: ·

Current license to practice as a registered professional nurse in the Commonwealth of Massachusetts. · Current Basic Life Support Certification · Valid Massachusetts driver’s license

Experience: ·

Minimum 3 years Hospice Experience · Inpatient hospice experience preferred · Hospice or end of life care experience required

Skills, Knowledge & abilities: · Must be sensitive to the needs of terminally ill patients and families and one's own feelings about dying and death. · Must demonstrate the ability to interact in a positive and courteous manner with patients, families, other employees, hospital staff and outside vendors. · The ability to work both independently and as part of a team are essential. · Demonstrates excellent observation and problem solving/critical thinking skills. · Well-developed verbal and written communication skills; essential computer skills to work with EMR. · Self-directed and able to work with minimal supervision · Shows strong ability to organize and prioritize workload independently; nursing skills per competency checklist.

Pay Range:

$87,000.00 USD - $128,900.00 USD

The pay range listed for this position is the annual base salary range the organization reasonably and in good faith expects to pay for this position at this time. Actual compensation is determined based on several factors, that may include seniority, education, training, relevant experience, relevant certifications, geographyof work location, job responsibilities, or other applicable factors permissible by law.
