Job summary

Advocate Health is seeking a Hospital at Home - Cancer Navigator to coordinate referrals and enable safe, timely transitions to home-based care. The role requires clinical licensure (RN/PT/OT/SLP) and at least 2 years in healthcare.

You will educate families on home care options, work with hospital teams, and coordinate with external partners to support patient discharge planning. The position is on-site, with 40 hours per week and travel to multiple site locations as needed.

Qualifications

• RN, PT, OT or SLP licensed in the practicing state.
• Minimum 2 years of healthcare experience.
• Excellent communication and clinical assessment skills.
• Ability to educate patients and families about home and post-acute care.
• Coordinate with hospital teams and community partners.

Responsibilities

• Coordinate transfer to Hospital at Home program and ensure orders.
• Educate patients and families about home care and hospice options.
• Collaborate with hospital units and social workers on complex cases.
• Serve as the communication link between patients, providers, and partners.
• Assess patient care needs including skin, cognitive, and mobility.
