Grace Hospice MN seeks a compassionate RN or LPN to join our hospice team for weekend and after-hours shifts, delivering skilled nursing visits in patients' homes and care settings. You will collaborate with the interdisciplinary team, observe changes in condition, educate families, manage symptoms, and advocate for comfort, dignity, and high-quality end-of-life care.
A Minnesota license and a minimum of three years’ nursing experience are required; weekend availability is essential.
If you are looking for a meaningful nursing career where you can make a difference one life at a time, Grace Hospice may be the right place for you.
At Grace Hospice, we provide professional care with a personal touch. Our focus is on the patient and family—not the disease—with an emphasis on comfort, dignity, quality of life, and compassionate end-of-life care.
As a nonprofit hospice organization, Grace Hospice remains grounded in the original roots of hospice care by supporting the spiritual, emotional, and physical needs of patients and families throughout the end-of-life journey.
We are seeking a compassionate, dependable Registered Nurse (RN) or Licensed Practical Nurse (LPN) to join our hospice team and provide skilled nursing support to patients and families during weekend and after-hours shifts.
LPN: $28.00–$35.00 per hour
RN: $39.90–$44.23 per hour
The Hospice RN/LPN works collaboratively with the triage nurse and interdisciplinary hospice team to provide skilled nursing visits to patients and families requiring after-hours support.
This position responds to patient needs in the home or care setting, identifies and reports changes in condition, assists with symptom management, provides patient and caregiver education, and communicates with the hospice care team to help ensure patients and families receive timely and compassionate care.
Responsibilities are performed within the nurse's applicable scope of practice, licensure, and Grace Hospice policies and procedures.
We anticipate the needs of our patients and families and strive to deliver exceptional care by going above and beyond expectations.
We distinguish ourselves by being compassionate, considerate, respectful, and generous in the way we care for patients, families, and one another.
We work respectfully and collaboratively to create the best possible end-of-life experience for our patients and families while supporting a positive work environment for our team.
At Grace Hospice, our nurses have the opportunity to provide care that is deeply meaningful. You will support patients and families during some of life's most important moments while working alongside a compassionate interdisciplinary team committed to excellence, kindness, teamwork, and dignity.
