Hope Health Care Limited in the Dallas area is seeking a Weekend On-Call RN for Hospice. The role focuses on supporting individuals and families facing terminal illness, delivering compassionate end-of-life care during weekend on-call shifts.

You will perform on-call visits starting Friday 5pm through Monday 8:30am, coordinate field visits, educate patients and caregivers, and uphold the company's values of dignity, compassion, and professionalism while ensuring comfort and excellence in care.

Formación

• Degree in Nursing is required.
• Valid Texas RN license is required.
• BLS certification is required.

Responsabilidades

• The weekend schedule begins at 5pm on Friday and ends at 8:30am Monday morning.
• Perform on-call PRN visits.
• Perform and coordinate field visits with the nursing team as needed by the patient's plan of care and in an effort to support their family.
• Train and educate patients, family/caregivers, and facility staff on appropriate usage of medications, DME, and supplies as well as the disease progression.
• Ensure concerns are addressed and the patient is comfortable - deliver excellence in patient care and attention.
• Understand the hospice philosophy, pain management, and the needs of the terminally ill and be able to convey that in language the patient and family understand.
• Demonstrate excellent interpersonal and communication skills.
• Demonstrate Hope Health Care's agency values - compassion, dignity, integrity, professionalism, collaboration, balance, growth and ethical behavior.

Your primary focus will be to support individuals and families who are facing the challenges that arise from a diagnosis that is terminal and at the end of life. Qualifications:

• Degree in Nursing.


• Valid RN Texas License.


• At least 1 year of Hospice experience (preferred).


• Valid Texas Driver's License and Proof of Auto Insurance.


• Reliable Transportation.


• Valid BLS.


• Successful completion of Background screening including employment verification.
