Job summary

Care Initiatives Hospice is seeking a PRN RN Hospice Visit Nurse to provide compassionate patient care in Council Bluffs, Avoca, Dunlap, and surrounding communities. You will collaborate with physicians and interdisciplinary teams to implement personalized care plans and support patients and families at home or in care facilities.

Qualified candidates hold an RN license with at least two years of nursing experience and CPR certification.

Qualifications

• RN with a minimum of two years of recent clinical nursing experience.
• Current, valid CPR certification.
• Demonstrated patience and a supportive and sensitive approach to providing patient, family, and caregiver support.
• Willingness and ability to provide care in facility and patient home settings.

Responsibilities

• Collaborate with the physician to oversee and implement personalized care plans, ensuring the active involvement of interdisciplinary teams to achieve the optimal outcomes for each patient
• Provide consistent, high-quality patient care to ensure an exceptional and seamless patient experience
• Perform initial and comprehensive assessments on all patients in accordance with policies and procedures
• Provides high-quality patient care while ensuring clear, concise, and accurate clinical documentation
• Assesses the need for plan of care updates while keeping the patient and family involved in care planning decisions
