Qualifications

Responsibilities

• Serve as case manager for hospitalized patients and manage GIP patients when appropriate.
• Navigate EMR systems to locate documentation for hospice admission.
• Conduct goals of care meetings with patients and families about hospice programs.
• Collaborate with community facilities to educate referral sources.
• Perform follow-up visits and calls after informational visits.
• Maintain and grow referral sources with hospitals and nursing homes.
• Document visits and communications with patients, families, and sources.
• Coordinate transition of care from hospital to home or admission to hospice, incl. consents.
• Mentor and train new RNs for high-acuity needs.
• Attend interdisciplinary care meetings and rounds.
• Establish relationships with hospital case managers, social workers, and physicians.

Position Overview

The Hospice Transition Nurse is a registered nurse who serves as a resource for patients, family members, and staff of hospitals, physicians, and nursing homes. The primary focus is sustaining and increasing referral sources from hospitals, nursing homes, and community partners by providing education on St. Croix Hospice programs and services.

Essential Functions and Skills

• Acts as a case manager for hospitalized patients and manages GIP patients when appropriate.
• Serves as a resource to the hospital and navigates EMR systems to locate documentation needed to support hospice admission.
• Conducts initial goals of care meetings with patients and families to inform them about St. Croix Hospice programs and services.
• Collaborates with business development in the community by providing education to facilities and referral sources.
• Performs follow‑up meetings and phone calls from informational visits.
• Sustains and increases referral sources with hospitals, nursing homes, and other community sources.
• Documents visits and phone calls with patients, families, and referral sources.
• Assists with transition of care for patients discharged from the hospital to home or admitted to St. Croix Hospice, coordinating with the admission nurse, Hospice Medical Director, comfort medications, and DME needs, and completing hospice admission consents.
• Mentors and trains new Registered Nurses for high‑acuity patient needs.
• Attends interdisciplinary care meetings regarding patients and families in the hospital.
• Establishes and maintains relationships with hospital case managers, social workers, and physicians, including attending any necessary daily rounds.
• Other duties as assigned, including routine visits, admissions, and clinical tasks.

Qualifications

• Current Registered Nurse (RN) license in the state(s) of practice.
• Ability to pass DHS background study.
• Valid state driver’s license, access to an automobile in good working order, automobile insurance in accordance with state and organizational requirements, and an acceptable driving record.

Preferred Qualifications

• Graduate of an accredited school of nursing. Associate degree required. Bachelor’s degree preferred.
• 1+ years of recent experience in hospice or home care.
• 2+ years of experience in nursing.
