Job summary

AccentCare, Inc. is seeking a Hospice Team Physician to provide clinical leadership and medical expertise within an interdisciplinary hospice care team, supporting terminally ill patients and collaborating with attending physicians to ensure high-quality, patient-centered care.

This hybrid, 8 hours/week position includes on-call coverage and teamwork with nurses, social workers, and chaplains. MD/DO license required; board certification in Hospice & Palliative Medicine preferred.

Qualifications

• Must be a licensed MD or DO in the practicing state.
• Active DEA license.
• Strong communication and collaboration with interdisciplinary teams.
• Ability to provide end-of-life care with sound clinical judgment.
• Board certification in Hospice and Palliative Medicine preferred.
• Five+ years of clinical experience preferred.

Responsibilities

• Serve as clinical leader within IDG and contribute to care planning.
• Provide medical care for hospice patients, including symptom management.
• Develop and update individualized Plans of Care.
• Complete certifications, recertifications, and face-to-face attestations.
• Collaborate with Attending Physicians and families on care plans.
• Participate in on-call coverage for 24/7 physician availability.
• Support hospice admissions, recertifications, revocations, and discharges.
• Maintain accurate, timely, compliant medical documentation.
