Position Summary
The Hospice Team Coordinator models Compassus values by coordinating and overseeing hospice care services. The role ensures accurate record‑keeping, facilitates communication among team members, and maintains regulatory compliance.
Responsibilities
- Accurately enter and manage patient data, including referral intake, admission details, and demographic updates.
- Ensure all admission documentation is complete, signed, dated, and includes required physician certifications.
- Upload documents into the HCHB system and maintain up‑to‑date patient records.
- Generate reports to demonstrate regulatory compliance and add coordination notes as needed.
- Monitor and report on durable medical equipment and medical supplies orders.
- Facilitate timely return of physician orders and coordinate their processing.
- Verify, upload, and organize paper‑based visit records into the EMR and HCHB systems.
- Confirm that the level of care matches billable levels to ensure accurate billing.
- Collect and upload decertification paperwork into patient records.
- Notify healthcare providers of patient discharge through coordination notes.
- Initiate requests for medical records and hospital inpatient charts and process provider notifications.
- Facilitate coordination among interdisciplinary team members for holistic patient care.
- Act as central point of contact for the hospice team, patients, and families, ensuring clear communication.
- Handle incoming telephone calls, take accurate messages, and transfer calls appropriately.
- Manage telephone systems and switch to secondary system when necessary.
- Assist the Hospice Director with pre‑ and post‑interdisciplinary team activities, including preparing agendas and updating plans of care.
- Prepare patient binders and ensure essential information is organized.
- Perform additional duties as assigned.
Qualifications
- High school diploma or GED required.
- Minimum two (2) years of experience in medical records, medical office, or business office support.
Skills
- Mathematical: add, subtract, multiply, and divide in all units; compute rate, ratio, and percentage.
- Language: read, analyze, and interpret general business periodicals, professional journals, technical procedures, and governmental regulations; write reports, business correspondence, and procedure manuals; present information and respond to questions from leaders, team members, investors, and external parties.
- Strong written and verbal communication; effective telephone skills; high integrity with confidential information.
- Proficiency in English; ability to understand, read, write, and speak; effective communication with physicians, nurses, and other healthcare personnel.
Physical Demands and Work Environment
The role requires regular standing, walking, and manual dexterity, with the ability to lift and move objects up to 25 pounds. Visual acuity for close and distance vision, color and peripheral vision, depth perception, and focus adjustment is necessary. Adaptations can be made to accommodate disabilities.
