Job summary

Transitions Hospice in Peoria, IL seeks a Team Assistant & Volunteer Coordinator to act as the operational backbone of the branch, ensuring clinical teams run smoothly, records stay accurate, and the volunteer program thrives.

The role involves scheduling, data entry, managing orders and care documentation, and supporting IDG meetings while developing and supervising a robust volunteer program to meet patient and family needs.

Qualifications

  • Bachelor’s degree preferred; advanced degree a plus.
  • 2+ years in healthcare setting.
  • Hospice and/or volunteer coordination experience preferred.
  • Strong attention to detail and organizational skills.
  • Comfortable working independently and with teams.

Responsibilities

  • Scheduling, faxing, phone calls, and day-to-day requests support.
  • Data entry of patient information and medical records maintenance.
  • Managing orders, plan of care documentation, and discharge paperwork.
  • Coordinate IDG meetings and serve as scribe for documentation.
  • Developing and maintaining the volunteer training/orientation program.
  • Recruiting and coordinating volunteers through yearly sessions.
  • Establishing volunteer service needs with patients and care team.
  • Supervising and evaluating volunteers; maintaining personnel files and stats.
  • Coordinating ongoing volunteer education meetings and support.
