Job summary

Caretenders Hospice, part of LHC Group within the Optum family, seeks a Home and Community Based Services Aide to assist clients at home with personal care and daily living activities.

The role includes bathing, grooming, meal prep, transportation to appointments, and documenting client status using EVV; Ohio STNA/Medicare HHA qualifications may apply. CPR readiness and flexible hours are valued, with comprehensive benefits.

Qualifications

Responsibilities

  • Provide personal care and assist with daily living activities (bathing, grooming, dressing, ambulation, reminders).
  • Assist with household tasks, meals, and accompany clients to appointments.
  • Monitor client condition, vital signs, and incidents; document observations with EVV.
  • Maintain safety and operate medical equipment as needed; respect diversity and confidentiality.
  • Adapt to home environments and support clients regardless of background.

As the Home and Community Bases Services Aide, you will provide support, assistance with personal hygiene and household functions for an individual to be able to remain in their own home.

Primary Responsibilities:
  • Provide personal care and assist with daily living activities such as bathing, grooming, dressing, ambulation, and medication reminders
  • Support household tasks, meal preparation, and accompany clients to appointments or errands as needed
  • Monitor and document client condition, vital signs, and incidents; maintain confidentiality and use EVV system
  • Ensure a safe environment, operate medical equipment properly, and respond promptly to client needs
  • Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client

Required Qualification:
  • Current driver’s license, vehicle insurance, and reliable transportation or access to public transportation

State-Specific Requirements:

Category I and II

  • OH: No Additional Requirements

Category III and IV:

  • OH: Must have successfully completed an approved Ohio Department of Health State Tested Nurse Aide (STNA) training program and competency, OR meet the qualifications to be Medicare Certified Home Health Aide, OR have at least one (1) year supervised employment experience as a Home Health Aide or Nurse Aide and has successfully completed a competency evaluation, OR has successfully completed a Council on Aging Learning Advantages (COALA) Home Health training program, a certified vocational or other sixty (60) hour training and competency evaluation program that meets the requirements listed in rule 173-39- 02.11(C)(3)(a)(v)(b)
Preferred Qualification:
  • Current CPR certification or ability to complete within 90 days of hire
  • 6+ months of home care experience
  • Ability to work flexible hours and independently

The hourly pay for this role will range from $16.75 - $25.10 per hour based on full-time employment. We comply with all minimum wage laws as applicable.
