Chapters Health System is seeking a Social Worker (SW) to provide psychosocial support and coordinate social services for patients and families across home health, hospice, and palliative care settings.

The role requires a CSWE-accredited MSW and healthcare experience, with ability to travel to off-site locations. Strong documentation, communication, and teamwork are essential to support comprehensive care plans.

Qualifications

• Master’s degree in Social Work (CSWE accredited) is required.
• Minimum of one year of healthcare or hospital experience.
• Experience with adult, pediatric and bereavement populations in clinical settings.

Responsabilités

• Performs and documents psychosocial assessments for patients and families.
• Empowers patients and families to negotiate with resource providers and make informed decisions.
• Maintains timely, complete, accurate, legible documentation.
• Collaborates with interdisciplinary team to ensure continuity of care.
• Supports hospice, palliative, and home health services as needed.
• Participates in IDG meetings and planning for Plan of Care.
