Job summary

Amedisys Home Health is seeking a Part-time Medical Social Worker in Savannah, MO to support patients and families with health-related social needs. You will provide social work services in the plan of care, educate families, and coordinate transfers and discharges from the care center.

The role requires a Master’s or Doctorate in Social Work (CSWE-accredited), at least one year of health care social work experience, and current CPR certification.

Qualifications

• Master's or Doctorate in Social Work from a CSWE-accredited program.
• At least one year of social work experience in a health care setting.
• Current CPR certification.

Responsibilities

• Assist patients and families with health-related financial, social and emotional concerns and coping skills.
• Provide social work services in accordance with the plan of care.
• Ensure changes in the patient's clinical status are communicated to the health care team promptly.
• Educate patient and family on social work interventions and treatment plan.
• Perform care planning, transfer and discharge tasks.
• Identify health risks in the patient environment and coordinate appropriate interventions.
