Job summary

MJHS is a not-for-profit health system delivering hospice and palliative care across the Greater New York area. You will provide psychosocial support to Hospice patients and their families in inpatient and home settings, and coordinate with the hospice IDG and nursing staff.

Qualifications

• Graduate from an accredited School of Social Work.
• Minimum of one year experience in Social Work in a healthcare setting.
• Knowledge of Word and Excel preferred.

Responsibilities

• Provide psychosocial support to Hospice patients and their families.
• Serve as social work member of hospice IDG and liaison between staff.
• Develop social work plan of care, goals and recommendations for assigned patients.
• Develop discharge plans when patients are admitted to inpatient facility.
