Job summary

Health Wealth Safe, Inc. is seeking a Social Worker to provide psychosocial support and counseling to patients and families facing terminal illness. The role involves collaborating with an interdisciplinary team to develop care plans, assess needs, and guide on benefits.

You will assist with grief, loss, and emotional stresses while coordinating resources and documenting care. The position requires an MSW, current licensure, and a valid driver’s license, with preference for 1 year of social work
