Job summary

Heart of Hospice, part of LHC Group, is seeking a Social Worker to provide medical social services under the direction of a physician and interdisciplinary group to support patients and families.

You will collaborate with the care team to understand social factors affecting health, contribute to a comprehensive Plan of Care, and guide the team on community resources and support services for patient well-being.

Qualifications

• Master's Degree in Social Work from a CSWE-accredited program.
• Current Driver's License, vehicle insurance, and access to dependable transportation.
• CPR certification or ability to complete within 90 days of hire.
• Licensed Social Worker in the state of residence.

Responsibilities

• Assists patient, family, physician and care team to understand personal, emotional and social factors related to health status.
• Contributes to development of a comprehensive Plan of Care for patients daily.

As the Social Worker, provides medical social services under the direction of a physician and Interdisciplinary Group to assist in the understanding of significant social and emotional factors related to the patient's health status and in development of coping mechanisms.

Primary Responsibilities:

• Assists the patient, significant others, physician and health care team staff to understand significant personal, emotional, environmental and social factors related to the patient's health status on an as needed basis
• Contributes as a health care team member to the development of a comprehensive, integrated Plan of Care for patients on a daily basis
• Instructs health care team members on community resources available to assist patients on a as needed basis

Required Qualifications:

• Master's Degree from a school of Social Work accredited by the Council of Social Work
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation

Pay is based on several factors including but not limited to local labor markets, education, work experience, certifications, etc. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements). The salary for this role will range from $56,440 to $84,660 annually based on full-time employment. We comply with all minimum wage laws as applicable.

Preferred Qualifications:

• 1+ years of social work experience in a health care setting
• Current CPR certification or ability to complete within 90 days of hire
• Licensed Social Worker in the state of residence
