Phoenix Home Care in Hutchinson, KS is seeking a Hospice Social Worker to provide compassionate in-home counseling and support for patients in the final stage of life and their families. You will collaborate with the care team, assist with advance directives, and coordinate resources to improve quality of life.

• Schedule: Monday–Friday | 8:00 AM–5:00 PM
• Hutchinson, KS and surrounding areas

Hospice Social Workers offer care and assistance to individuals in the final stage of life, as well as their families and close ones. Their purpose is to provide emotional and practical support, with the goal of enhancing the well‑being of those confronting end‑of‑life challenges. Hospice social workers collaborate closely with healthcare teams to organize treatments, services, and available resources, ultimately aiming to enhance a patient's overall quality of life.

Requirements

• Master’s in Social Work (MSW) or LCSW required
• Hospice experience preferred
• Strong communication and interpersonal skills
• Ability to work independently and as part of a team
• Professional, dependable, and organized
