Job summary

Amedisys is seeking a Social Worker Hospice (PRN Per Diem) to support patients and families in Mystic and surrounding areas. Masters level, licensed, with a flexible weekly schedule around 10 hours or less, enabling meaningful home-based care.

Responsibilities include assessing psychosocial needs, coordinating care with the team, and educating families. Competitive pay ranges from $38.00 to $45.00 per hour, with comprehensive benefits available.

Qualifications

• Master's or Doctorate in Social Work from a CSWE-accredited program.
• One year of social work experience in a health care setting.
• Current CPR certification.

Responsibilities

• Assists the patient and family with health related financial, social and emotional concerns and assists with effective coping skills.
• Provides social work services in accordance with the plan of care.
• Ensures that changes in the patient's clinical status are communicated to other healthcare team members and the care center office staff in a timely manner.
• Educates the patient and family members and other team members on medical social work interventions and treatment plan.
• Performs care planning, transfer and discharge functions.
• Identifies risks or threats to the patient's health in the patient's environment and promptly implements appropriate interventions to reduce risks.
• Other duties as assigned.
