Job summary

The Parker Jewish Institute for Health Care and Rehabilitation in New Hyde Park, NY, is seeking a full-time Hospice Social Worker (LMSW) to join our interdisciplinary team. Qualifications

• Master's degree in Social Work (MSW) required.
• Active NYS Social Work License.

Responsibilities

• Conduct psychosocial, emotional, spiritual and bereavement assessments.
• Develop and implement IDG care plans with patient and family.
• Advocate for patients and families; deliver field and in-patient visits in Queens and Nassau counties.
