Job summary

You will assess, plan, and implement care with an interdisciplinary team, educate on advanced directives, support coping, coordinate with physicians, and participate in quality improvement and on-call duties.

Qualifications

• MSW from CSWE-accredited program.
• State licensure required.
• Minimum of 1 year experience in health care; hospice experience preferred.
• Strong verbal and written communication and organizational skills.

Responsibilities

• Assesses psychosocial status of patients and families/caregivers in relation to terminal illness and plans interventions.
• Carries out social evaluations and counsels patients and families; provides crisis intervention.
• Educates interdisciplinary group on cultural needs and advanced directives.
• Participates in developing individualized plan of care and attends interdisciplinary meetings.
• Communicates with physicians and coordinates care plans; maintains accurate clinical notes.
• Assists with funeral, financial, legal and healthcare decision planning; ensures safety of environment.
• Attends on-call duties and supports RN Case Manager to arrange equipment and services.
