• Must have a master’s degree in social work and hold a Social Worker License in Georgia.
• Two years of supervised experience in health care; hospice experience preferred.
• Certified Hospice and Palliative Social Worker desirable.
• Willingness to travel within Bristol Hospice service area.

Responsibilities

• Assess the psychosocial status of patients and families related to terminal illness.
• Perform social evaluations including family dynamics and caregiver abilities.
• Counsel patients and families; provide crisis intervention when needed.
• Educate interdisciplinary team on culture-specific needs and communication.
