Job summary

Medcare Hospice in Miami, FL is seeking a compassionate Social Worker to provide and coordinate psychosocial services for patients and families at the end of life. You will empower decisions, collaborate with the care team, and support bereavement needs.

Responsibilities include assessments, documentation, and participation in interdisciplinary meetings to ensure holistic, patient-centered care. A CSWE-accredited MSW and experience with EMR/EHR are preferred.

Qualifications

• Master’s degree or above in Social Work (CSWE accredited).
• FL LCSW license preferred.
• Minimum 1 year of health care or hospital experience.
• Hospice/palliative care experience preferred.
• Experience with EMR/EHR systems.
• Excellent communication and bilingual in Spanish preferred.

Responsibilities

• Perform psychosocial assessments.
• Empower patients and families to make informed decisions.
• Assist clinical staff with family dynamics and care plans.
• Maintain accurate and timely documentation.
• Collaborate with other professionals to promote quality care.
• Participate in interdisciplinary meetings and provide education.
• Provide psychosocial support and bereavement services.
• Attend Interdisciplinary Group meetings.
• Evaluate bereavement needs and coordinate with Bereavement staff.
• Perform other duties as assigned.
