Hospice of New Mexico is seeking a Licensed Medical Social Worker to provide compassionate social work services to hospice patients and families in Albuquerque. This role focuses on counseling and guiding emotional well‑being, with a Monday–Friday schedule from 8:00 am to 4:30 pm.

Required: MSW, LMSW license, NM license, and healthcare social work experience; hospice experience preferred. Compensation ranges from $39 to $40 per hour, with comprehensive benefits included.

Formación

• Master of Social Work from an accredited school.
• Licensed social worker (State Board of Social Work).
• Current driver's license, vehicle insurance, and access to a dependable vehicle.
• Minimum 1 year healthcare social work experience; hospice experience preferred.
• Current unencumbered NM Social Worker license.

Responsabilidades

• Assess psychosocial status and communicate with the RN Case Manager.
• Evaluate medical needs, home situations, resources, and community supports.
• Implement personalized social work care plans with Bereavement Coordinator.
• Attend interdisciplinary meetings and help shape care plans.
• Provide empathetic counseling to patients and families.
