Job summary

Enhabit Home Health & Hospice is seeking a Licensed Social Worker to support patients in home-based care across New Mexico. You will assess needs, coordinate services, and educate patients and families within the established plan of care.

Ideal candidates hold a BS in social work, are licensed in the state, and have at least one year of healthcare social work experience. You’ll join a team dedicated to compassionate, patient-centered care and ongoing professional development.

Qualifications

• Must be organized and detail-oriented.
• Must be able to communicate effectively both orally and in writing.
• Must be capable of making independent decisions relevant to the patient’s needs.
• Must be capable of prioritizing and handling multiple tasks of critical importance simultaneously.
• Must function well in atmosphere of stress and interruption.

Responsibilities

• Administer social services to patients requiring intermittent professional services and teach the patient, family, and other members of the health care team. Perform services in accordance with the physician’s orders and the established plan of care (POC).
