Job summary

Pallatus Health is seeking a licensed social worker to provide emotional support, counseling, and care coordination for hospice patients and their families in Los Angeles, CA. You will collaborate with an interdisciplinary team to address emotional, social, and practical needs and help families navigate end-of-life decisions.

The role includes grief support, advance directive discussions, community resource linkage, and continuation of patient care through crisis intervention and bereavement

Qualifications

• Master's degree in Social Work (MSW) from an accredited program.
• Current state licensure (LCSW, LMSW, or equivalent).
• Minimum 2 years of clinical social work experience.
• Experience with grief counseling and bereavement support.
• Strong assessment and crisis intervention skills.
• Excellent communication and active listening abilities.
• Cultural sensitivity and ability to work with diverse populations.
• Valid driver's license and reliable transportation for home visits.
• Computer proficiency and electronic health record experience.

Responsibilities

• Conduct psychosocial assessments and develop care plans addressing emotional and social needs
• Provide individual and family counseling to help cope with illness, loss, and grief
• Facilitate advance directive discussions and end-of-life planning conversations
• Connect families with community resources, financial assistance, and support services
• Lead grief support groups and provide bereavement follow-up services
• Collaborate with interdisciplinary team to ensure comprehensive patient care
• Document patient interactions and maintain compliance with regulatory standards
• Provide crisis intervention and emergency psychosocial support
• Educate families about the dying process and what to expect
• Coordinate with nursing homes, hospitals, and other care facilities
