Job summary

Benaroya Services at Kline Galland is seeking a Full-Time Social Worker to join our Hospice and Palliative Care team in Seattle. This role focuses on psychosocial assessments, care planning with an interdisciplinary team, crisis intervention, and resource coordination for patients and families facing end-of-life care.

You will work across settings including private residences, Adult Family Homes, independent and assisted living communities, and Skilled Nursing Facilities in King County.

Qualifications

• Master’s Degree MSW from an accredited program.
• Minimum 2 years of social work experience.
• 1 year in Hospice and/or Home Health preferred.
• Strong verbal and written communication skills.
• Proficiency in computer charting and documentation systems.
• Valid driver’s license, auto insurance, and reliable transportation.

Responsibilities

• Conduct comprehensive psychosocial assessments.
• Develop and implement plans of care in collaboration with the interdisciplinary team.
• Provide crisis intervention and therapeutic support.
• Coordinate resource referrals and community services.
• Support patients and families navigating end-of-life care.
• Provide care across multiple settings, including private residences, adult family homes, independent & assisted living communities, and skilled nursing facilities.
