Job summary

Compassus is seeking a Hospice Social Worker to provide psychosocial assessments, counseling, and care coordination within interdisciplinary teams, enhancing patients’ coping and quality of life.

You will collaborate with IDT, explain Medicare benefits, support families, and facilitate long-term care planning while upholding Compassus values and ethical standards.

Qualifications

• Master of Social Work (MSW) from a CSWE-accredited program with at least 1 year of healthcare experience.
• Candidates with a BSW in social work and 1 year of healthcare experience may be considered for locations with MSW supervision.
• Must hold state licensure as required for the state of practice.

Responsibilities

• Explains hospice services and Medicare benefits to patients and families; obtains Informed Consent and Election of Benefits documents as requested.
• Submits appropriate documentation and paperwork to facilities at the end of patient visits in a timely manner according to policy.
• Participates as a member of the IDT, including development and implementation of the plan of care.
• Provides psycho-social support and counseling services to the patient and family; collaborates with external counseling resources as needed.
• Assists in identifying the need for intervention of other IDT members.
• Effectively communicates patient and family needs to IDT.
• Collaborates with IDT to coordinate psycho-social care and support for patient and family for continuity and quality.
• Maintains updated, comprehensive care plans reflecting current problems, goals and interventions.
• Respects family environment and beliefs; remains nondiscriminatory in all aspects of care.
• Completes initial and ongoing psycho-social assessments; addresses financial, insurance, living arrangements, caregiver stressors, pre-bereavement and community needs.
• Facilitates placement in long-term care as warranted and monitors adjustment.
• Participates in care planning meetings at long-term care facilities.
• Supports advance planning including living will/POA, funeral arrangements, memorial services, and body/organ donation.
• Performs other duties as assigned.

Position Summary

The Hospice Social Worker provides preventive, educational, and evaluative and treatment services to meet the psychosocial needs of patients and their families, to help them improve, restore, and maintain their maximum level of coping. S/he utilizes community resources and the interdisciplinary team (IDT) to aid in this process.

Education And/or Experience

• Master of Social Work (MSW) from a CSWE accredited program accompanied by at least 1 year of healthcare experience
• Candidates who have a bachelor’s degree in social work or related field and 1 year of healthcare experience will be considered for locations who have an MSW available to supervise.
• Candidates must hold state licensure as required and applicable for the state in which you practice.

Skills

• Language Skills: Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations.
