Interim HealthCare Inc. is seeking a Hospice Medical Social Worker in Indiana to provide patient counseling and crisis support, helping families access community resources and plan their care journey.

This role focuses on compassionate, person-centered hospice services. Qualified candidates hold an MSW with an unrestricted LSW license (LISW preferred) and at least one year of hospice/palliative care experience, plus reliable transportation to care sites.

Qualifications

• Current unrestricted license to practice as a Licensed Social Worker (LSW) in the state; LISW preferred.
• One year of experience as a Licensed Social Worker in hospice/palliative care or similar setting.
• Reliable transportation to/from care sites or work locations.

As a Hospice Medical Social Worker, You Will

To qualify as a Hospice Medical Social Worker with us, you will need:

• Education: Master of Social Work (MSW) required
• Licensure: Current unrestricted license to practice as a Licensed Social Worker (LSW) in the state associated with this position required; Licensed Independent Social Worker (LISW) preferred
• One (1) year of experience practicing as a Licensed Social Worker in a hospice/palliative care or similar setting
• Reliable transportation to/from care sites/ or work locations
